Social Worker - Changing Futures Team (Adults Social Care)

£35.48 per hour
Hull City Council is seeking an experienced Social Worker to join the Changing Futures Team within Adult Social Care.
The role supports adults experiencing multiple disadvantages, including homelessness, substance misuse, mental health needs, domestic abuse, and contact with the criminal justice system.
The service uses a trauma-informed, strengths-based approach focused on improving outcomes and promoting independence.
Key responsibilities:
Manage a caseload of adults with complex and multiple needs
Work with individuals experiencing homelessness and rough sleeping
Support people with substance misuse, mental health, and justice system involvement
Undertake Care Act assessments, support planning, and reviews
Work in a multi-agency team including housing, mental health, probation, and community services
Apply trauma-informed and strengths-based practice approaches
Contribute to safeguarding and positive risk management
Support implementation of relevant legislation including Care Act 2014 and Mental Capacity Act 2005
Promote co-production and community-based solutions
