Job Title: Social Worker - Preparing for Adulthood Team
Location: Bury, Greater Manchester
Salary/Rate: £30 per hour
Service Area: Adult Social Care
About the Role:
Are you passionate about supporting young people as they transition into adulthood? Bury Council's Preparing for Adulthood (PFA) Team is looking for an experienced Social Worker to join their dedicated team.
In this role, you will:
Support young people aged 14-25 with care needs, disabilities, and complex transitions
Hold a caseload and deliver high-quality, strengths-based, person-centred social work
Plan and deliver smooth, empowering transitions aligned with the Preparing for Adulthood national framework
Build strong partnerships with schools, colleges, health services, and multi-agency teams
Champion young people's voices and aspirations in all decisions
Contribute to quality assurance, performance oversight, and best practice development
About You:
We are looking for a compassionate and confident social worker who:
Has experience in adult social care, transitions, or disability services
Holds the relevant social work qualifications
Is solution-focused, organised, and collaborative
Is committed to improving outcomes for young people and families
Believes that transition is a journey, not just a process
