Southwark - Social Worker - Assessment & Intake Team
We are currently seeking a dedicated and experienced Social Worker to join our Assessment & Intake Team in Southwark. As a key member of our team, you will be responsible for conducting assessments and providing support to individuals and families in need within the community.
Key responsibilities and duties:
- Conducting comprehensive assessments to identify the needs and risks of service users.
- Developing and implementing care plans in collaboration with service users and their support networks.
- Providing guidance and support to service users to promote their well-being and independence.
- Working closely with other professionals and agencies to ensure coordinated and effective service delivery.
- Participating in case conferences and reviews to monitor progress and make necessary adjustments to care plans.
Essential qualifications and skills:
- Qualified Social Worker with a valid HCPC registration.
- Demonstrated experience in conducting assessments and working with vulnerable individuals and families.
- Strong communication and interpersonal skills to engage effectively with service users and stakeholders.
- Ability to work autonomously and as part of a multidisciplinary team.
- Sound knowledge of relevant legislation and policies related to social work practice.
