Join our dedicated Adults Community Team in Derbyshire as a Qualified Social Worker, where you will play a vital role in supporting adults to live fulfilling lives within their communities. In this position, you will work collaboratively with individuals, families, and other professionals to assess needs, develop care plans, and provide ongoing support tailored to each person’s unique circumstances.

Your key responsibilities will include:

  • Conducting comprehensive assessments of adult clients to determine their needs and eligibility for services.
  • Developing, implementing, and reviewing care plans in partnership with clients and their families.
  • Facilitating access to services and resources to promote independence and well-being.
  • Maintaining accurate and up-to-date records in accordance with legislation and best practices.
  • Participating in multi-disciplinary team meetings and collaborating with other agencies to ensure holistic support.
  • Providing guidance and support to less experienced team members and contributing to the professional development of the team.

The ideal candidate will possess the following qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Experience working within adult social care or community settings.
  • Strong assessment and analytical skills, with the ability to develop effective care plans.
  • Excellent communication and interpersonal skills, with a commitment to person-centered practice.
  • Knowledge of relevant legislation and policies affecting adult social care.
