My client in Havering is looking for part-time Social Worker (3 days per week) for their Review team.

This is within the Hospital Assessment and Community Review (HACR) service

The Hospital Assessment and Community Review (HACR) Service is seeking a dedicated and experienced Social Worker to join our Review Team on a part-time basis (3 days per week).

About the Service:

The Hospital Assessment and Community Review (HACR) Service brings together three specialist teams that work collaboratively to support Havering residents through their journey from hospital discharge to recovery and long-term independence. These teams include Hospital Discharge, the Review Team, and the Discharge to Assess Team.

About the Role:

As a Social Worker within the Review Team, you will play a vital role in supporting residents following hospital discharge, ensuring they receive the right level of care and support to maximise their independence and well-being.

Your responsibilities will include:

Completing Care Act Assessments for individuals with ongoing care and support needs following a Home First pathway.
Reviewing care and support plans to ensure residents changing needs continue to be met while promoting wellbeing, choice, independence, and safety.
Supporting residents through a range of rehabilitation pathways, including non-weight-bearing rehabilitation, specialist rehabilitation, slow-stream rehabilitation, and the Collar and Brace Aftercare Service (CBACS).
Working in partnership with Haverings Quality Assurance Team where concerns are identified within care homes or domiciliary care providers, helping to safeguard residents and promote high standards of care.

About You

We are looking for a fully qualified Social Worker (registered on SWE) who is passionate about enabling people to achieve the best possible outcomes while maintaining their independence. You will be committed to delivering person-centred practice, have excellent assessment and care planning skills, and enjoy working collaboratively within a multidisciplinary team.
