Adult's SW - LD Team - London Borough of Lambeth
We are currently seeking a dedicated and compassionate Social Worker to join the Adult's Learning Disabilities Team in the vibrant London Borough of Lambeth. As a key member of the team, you will be responsible for providing high-quality support and intervention to adults with learning disabilities, empowering them to lead fulfilling and independent lives.
Key responsibilities and duties:
- Assessing the needs of adults with learning disabilities and developing care plans tailored to their individual requirements.
- Working collaboratively with service users, their families, and multi-disciplinary teams to ensure effective support and safeguarding.
- Providing advocacy and guidance to empower service users in making informed decisions about their lives.
- Monitoring and reviewing care plans regularly to adapt to changing needs and ensure positive outcomes for service users.
- Participating in case conferences, reviews, and court proceedings as required, maintaining accurate and up-to-date records.
Essential qualifications and skills:
- Qualified Social Worker with a valid HCPC registration.
- Demonstrated experience working with adults with learning disabilities in a social care setting.
- Strong assessment, communication, and interpersonal skills to engage effectively with service users and stakeholders.
- Ability to work autonomously and as part of a team, demonstrating resilience and adaptability in a fast-paced environment.
- Commitment to continuous professional development and adherence to relevant legislation and best practice guidelines.

- Knowledge of relevant legislation, such as the Care Act and Mental Capacity Act.
