Adult's SW - Locality Team - Havering
We are currently seeking a dedicated and experienced Adult's Social Worker to join our Locality Team in Havering. As a key member of our team, you will be responsible for providing high-quality social work services to adults in the community, with a focus on promoting independence and improving their quality of life.
Key responsibilities and duties include:
- Undertaking assessments to determine the needs of adults and their carers
- Developing care and support plans in collaboration with service users and their families
- Providing advocacy and support to empower individuals to make informed decisions
- Working closely with multi-disciplinary teams and external agencies to coordinate services
- Participating in safeguarding procedures to ensure the welfare of vulnerable adults
The ideal candidate for this position will possess the following essential qualifications and skills:
- Qualified Social Worker with a relevant degree and HCPC registration
- Demonstrated experience in working with adults in a social care setting
- Strong assessment and analytical skills, with the ability to make evidence-based decisions
- Excellent communication and interpersonal skills to engage effectively with diverse individuals
- Ability to work autonomously and as part of a team in a fast-paced environment
