About the Role

We are currently recruiting for an experienced Qualified Social Worker to join the North & East Neighbourhoods Team in Bolton. This is a fantastic opportunity to work within a supportive, multi-disciplinary team delivering a joined-up approach to meeting the needs of local residents.

You will play a key role in supporting adults within the community, promoting independence, and delivering high-quality, person-centred care.

Key Responsibilities

  • Undertake strengths-based assessments under the Care Act 2014
  • Complete carers' assessments and develop comprehensive support plans
  • Support individuals in accessing universal and voluntary services
  • Manage a busy duty rota, handling referrals and providing advice at first point of contact
  • Work collaboratively within a multi-disciplinary team

Key Requirements

  • Qualified Social Worker with relevant registration
  • Strong knowledge of the Care Act 2014 and Mental Capacity Act 2005
  • Experience managing a caseload and delivering person-centred support
  • Ability to undertake safeguarding investigations
  • Experience in Continuing Healthcare assessments and multi-agency working (desirable)
  • Strong communication and decision-making skills

Additional Information

  • Hybrid working: expected to work in the office 3 days per week
  • Initial induction period will be primarily office-based
  • Flexible working hours between 8am - 7pm (approx. 7-hour working day)
  • Role involves visiting individuals in their homes and community settings across Bolton
  • Candidates must provide 3 years' references and a full employment history
