- Conducting thorough assessments of needs and risks to inform care planning.
- Developing and implementing intervention strategies in partnership with service users.
- Maintaining accurate and up-to-date case records in accordance with legislation and best practices.
- Participating in multi-disciplinary meetings and contributing to team discussions.
- Providing expert advice and support to colleagues and stakeholders.
To be successful in this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Demonstrable experience in assessment and care planning within a social work context.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent analytical and problem-solving abilities.
