Job Title: Move On Coordinator
Job Purpose
The Move On Coordinator is responsible for supporting clients to transition successfully from temporary, supported, or emergency accommodation into independent living. The role involves providing practical advice, coordinating move-on plans, liaising with housing providers and external agencies, and ensuring that clients sustain their tenancies and achieve positive outcomes.
Key Responsibilities
- Develop and implement personalised move-on plans for service users.
- Support clients in securing suitable permanent or longer-term accommodation.
- Conduct assessments to identify housing needs, risks, and support requirements.
- Liaise with local authorities, housing associations, landlords, and other partner agencies to facilitate move-on opportunities.
- Assist clients with tenancy applications, benefit claims, budgeting, and setting up utilities.
- Provide advice and guidance on tenancy rights and responsibilities.
- Maintain accurate client records, case notes, and performance data in accordance with organisational policies.
- Monitor clients' progress and provide ongoing support to help sustain tenancies.
- Work collaboratively with internal teams and external stakeholders to achieve positive outcomes for service users.
- Safeguard vulnerable adults and report any concerns in line with safeguarding procedures.
- Contribute to service development initiatives and continuous improvement activities.
Person Specification
Essential Requirements
- Experience working within housing, homelessness, supported accommodation, or a related field.
- Knowledge of housing legislation, welfare benefits, and tenancy sustainment practices.
- Strong case management and organisational skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively with vulnerable individuals from diverse backgrounds.
- Competence in maintaining accurate records and using case management systems.
- Understanding of safeguarding principles and procedures.
Desirable Requirements
- Relevant qualification in Housing, Social Care, Community Development, or a related discipline.
- Experience working with local authorities, housing associations, or support services.
- Knowledge of homelessness prevention and resettlement services.
