We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Adult Social Care team in Plymouth. As a key member of our team, you will be responsible for providing high-quality social work support to adults in need, promoting their well-being and independence.

Key responsibilities and duties:

  • Conducting assessments of individuals to determine their care and support needs
  • Developing and implementing care plans in collaboration with service users and their families
  • Providing advocacy and support to empower individuals to make informed decisions
  • Working closely with multi-disciplinary teams to ensure holistic and coordinated care
  • Participating in safeguarding procedures and interventions to protect vulnerable adults

Essential qualifications and skills:

  • Qualified Social Worker with a relevant degree and HCPC registration
  • Demonstrated experience in adult social care settings, preferably within a local authority
  • Strong assessment and analytical skills to identify needs and risks
  • Excellent communication and interpersonal skills to build rapport with service users
  • Ability to work autonomously and as part of a team in a fast-paced environment
