We are currently seeking a dedicated and experienced Social Worker to join our Cheshire West - SSW - Connected Peoples Team. As a key member of our team, you will be responsible for providing high-quality social work services to individuals and families in need within the Cheshire West area.
Key responsibilities and duties include:
- Conducting assessments and developing care plans for vulnerable individuals and families
- Providing support and intervention to promote positive outcomes for service users
- Working collaboratively with partner agencies to ensure holistic support
- Participating in case conferences and reviews to monitor progress and adjust plans as needed
- Advocating for the rights and best interests of service users
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with current registration with Social Work England
- Demonstrated experience in working with vulnerable individuals and families
- Strong assessment and analytical skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced and challenging environment
