Adult's SW - Transition Team - Sefton

We are currently seeking a dedicated and experienced Adult's Social Worker to join our Transition Team in Sefton. As a key member of the team, you will be responsible for providing high-quality social work support to adults undergoing transitions in their lives, ensuring their well-being and empowerment.

Key responsibilities and duties include:

  • Assessing the needs and risks of adults within the transition process
  • Developing and implementing care plans in collaboration with service users and their families
  • Providing advocacy and support to empower individuals to make informed decisions
  • Working closely with multi-disciplinary teams to coordinate services and support
  • Conducting regular reviews and assessments to monitor progress and adjust interventions as needed

The ideal candidate for this position will possess the following essential qualifications and skills:

  • Qualified Social Worker with a recognized social work qualification
  • Registered with Social Work England
  • Demonstrated experience in adult social work, particularly within transition services
  • Strong assessment, communication, and interpersonal skills
  • Ability to work effectively in a fast-paced and challenging environment

Additional requirements for this role include a valid driver's license and access to a vehicle for travel within the Sefton area.
