Worcestershire - Social Worker - Adults Community
We are currently seeking a dedicated and experienced Social Worker to join our team in Worcestershire, focusing on supporting adults within the community. As a key member of our Qualified Social Work team, you will play a crucial role in assessing, planning, and implementing care and support services for vulnerable adults.
Key responsibilities and duties:
- Conducting assessments to identify the needs of service users and develop care plans tailored to their individual circumstances.
- Working collaboratively with multi-disciplinary teams, service users, and their families to ensure holistic support and interventions.
- Providing advocacy and support to empower service users to make informed decisions about their care and well-being.
- Monitoring and reviewing care plans regularly to ensure they remain effective and meet the changing needs of service users.
- Participating in safeguarding procedures and taking appropriate action to protect vulnerable adults from harm or abuse.
Essential qualifications and skills:
- Qualified Social Worker with a recognized social work qualification and registration with Social Work England.
- Demonstrated experience in working with vulnerable adults in a community setting, with a strong understanding of relevant legislation and best practice.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with service users and their support networks.
- Strong assessment and decision-making abilities, with a focus on person-centered care and support planning.
- Ability to work autonomously, manage a complex caseload, and prioritize tasks effectively to meet deadlines.
