Stockton - Advance Practitioner - Children Services Team
Eden Brown Synergy is currently looking for an experienced & committed Advance Practitioner on Stockton on Tees Borough Council, offering an excellent opportunity to make a meaningful impact within a well-established Children Services Team.
Payrate: £34.45 p/h Umb
Duties and Responsibilities:
- Liaising/ Working close with partnership agencies to support the families and meet the children'/ young people's needs
- Preparing & implementing Care Plans
- Attending court and giving evidence when required
- Obtaining Interim Care Orders/Interim Supervision Orders
- Initiating Care Proceedings
- Completion of Care Proceedings
- Obtaining Special Guardianship Orders
- Producing Child Permanency Reports
- Reviewing Financial payments received by kinship carers/ or carers granted Residence Orders/Special Guardianship Orders
- Preparing looked after children review reports
- Initiating Personal Education Plans (PEP's)
- Producing Health Care Assessments and Health Plans
- Making referrals for Foster Placements
- Preparing children for placements
- Implementing Twin Tracking processes for children in care proceedings
