Team Description/Dynamic:
Join a locality-based multi-disciplinary team dedicated to making a real difference in the community. This role involves assessing and supporting clients with a range of health conditions in a collaborative setting.

Key Responsibilities:

  • Assess and make recommendations for major adaptations and complex equipment, including preparing sketches, reading plans, and providing detailed specifications for major works.
  • Manual handling assessments and guidance.
  • Work within a multi-professional team to conduct complex OT assessments, support planning, and reviews of individuals with diverse health conditions and their carers.
  • Demonstrate understanding of the legal social care framework and housing legislation.
  • Manage, plan, prioritise, and progress complex cases effectively.
  • Assess, recommend, and review simple and complex equipment, ensuring compliance with relevant legislation and policies.

Essential Requirements:

  1. Expert Knowledge & Skills: Strong experience in assessing and recommending major adaptations and complex equipment. Proficient in manual handling.
  2. Experience in Multi-Professional Teams: Experience in conducting holistic person-centred assessments, support planning, and reviews.
  3. Legal & Housing Framework Understanding: Solid knowledge of the legal social care framework and housing legislation.
  4. Case Management Expertise: Proven experience in managing and prioritising complex cases.
  5. Equipment & Safe Use Knowledge: Experience in assessing for and recommending both simple and complex equipment, with the ability to teach safe use and review effectiveness.

Essential Experience:

  • Excellent knowledge of specialist equipment, major adaptations, and manual handling.
  • Experience with OT duty and the ability to work autonomously.
  • Strong experience in a multi-disciplinary setting, with the ability to prioritise and manage workload effectively.

Qualifications:

  • Professional Occupational Therapy qualification as recognised by the HCPC.
