We are currently seeking an experienced Social Worker for a locum Advanced Practitioner role within a Continuing Healthcare (CHC) team on the Wirral. This is an excellent opportunity to take on a leadership-focused position while making a meaningful impact in a flexible, temporary assignment.

Key Responsibilities:

  • Undertake Continuing Healthcare (CHC) assessments and reviews
  • Apply the Mental Capacity Act (MCA) in practice
  • Supervise, mentor, and support team members
  • Deliver high-quality, person-centred care

About You:

  • Qualified and experienced Social Worker
  • Strong background in CHC assessments and reviews
  • Confident applying the Mental Capacity Act
  • Experience supervising or mentoring staff
  • Committed to delivering excellent, person-centred outcomes
  • SWE REGISTERED
