£38.38 per hour (Limited/Umbrella)
Hybrid Working: 2 days remote | 3 days office-based
We are currently seeking an experienced Social Worker for a locum Advanced Practitioner role within a Continuing Healthcare (CHC) team on the Wirral. This is an excellent opportunity to take on a leadership-focused position while making a meaningful impact in a flexible, temporary assignment.

Key Responsibilities:
- Undertake Continuing Healthcare (CHC) assessments and reviews
- Apply the Mental Capacity Act (MCA) in practice
- Supervise, mentor, and support team members
- Deliver high-quality, person-centred care
About You:
- Qualified and experienced Social Worker
- Strong background in CHC assessments and reviews
- Confident applying the Mental Capacity Act
- Experience supervising or mentoring staff
- Committed to delivering excellent, person-centred outcomes
- SWE REGISTERED
