Children's SW - Discharge Planning Team - Staffordshire

Key responsibilities include:
- Conducting comprehensive assessments to identify the needs of children and families.
- Developing and implementing discharge plans in partnership with families and other professionals.
- Providing support and guidance to families throughout the discharge process.
- Maintaining accurate records and documentation in accordance with local policies and procedures.
- Participating in multi-agency meetings to ensure coordinated care and support.
To be considered for this role, you must possess the following qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Current registration with Social Work England.
- Experience in children's services, particularly in discharge planning or similar settings.
- Strong assessment and analytical skills.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
