Staffordshire - QSW / SP - Discharge Planning Team
Eden Brown Synergy are currently looking for an experienced and motivated Qualified Social Worker / Senior Practitioner to join the Discharge Planning Team at Staffordshire County Council.
Pay Rate: Up to £33.30 per hour (Umbrella)
Location: ST16 2DH, Staffordshire
This is an excellent opportunity to join the Discharge Planning Team, supporting children, young people and families by ensuring safe, effective and timely discharge planning. You will work collaboratively with health professionals, partner agencies and families to coordinate assessments, care planning and appropriate support to achieve the best possible outcomes.
You will be responsible for managing a varied caseload, undertaking statutory assessments, safeguarding interventions and ensuring that discharge plans are child-centred, robust and delivered in line with statutory requirements.
Key Skills Required:
* Experience within Children's Services, Assessment, Safeguarding or Discharge Planning
* Strong safeguarding, assessment and care planning skills
* Experience working with multi-agency partners, including health professionals
* Ability to manage complex caseloads and make informed decisions
* Excellent communication, report writing and organisational skills
* Social Work England registration required
Essential Requirements:
* Social Work Qualification - Degree or equivalent
* Registered with Social Work England
* Previous experience within Children's Services
* Experience as a Qualified Social Worker or Senior Practitioner
