We are currently seeking a dedicated and compassionate individual to join our Leaving Care Team as a Stoke - AP - Leaving Care worker in the Care (Qualified) field. As part of this role, you will be responsible for providing support and guidance to young adults transitioning from care to independent living.

Key responsibilities and duties include:

  • Developing and implementing personalized care plans for each individual based on their specific needs and goals.
  • Offering emotional support and practical assistance to help young adults navigate challenges and build essential life skills.
  • Advocating for the rights and best interests of care leavers within the community and with external agencies.
  • Collaborating with other professionals, such as social workers and mental health specialists, to ensure comprehensive support for each individual.

Essential qualifications and skills for this role include:

  • Qualified Social Worker or relevant qualification in a related field.
  • Experience working with young people in care or leaving care, with a strong understanding of the challenges they may face.
  • Excellent communication and interpersonal skills to build trusting relationships with care leavers and support their development.
  • Ability to work effectively both independently and as part of a team, demonstrating empathy, patience, and resilience.

Additional requirements for this position may include a valid driver's license and access to a vehicle for travel to meet with care leavers in the community.
